Referring from primary care
If you are referring from primary care, please refer via Specialist & Referrals from your practice management system or alternatively you can email the referral to us at referral@aotearoadiabetescollective.org.nz.
Referring from Secondary Care
If you are referring from a specialist service / hospital department please send a referral to us at referral@aotearoadiabetescollective.org.nz
