We welcome referrals from people with diabetes and their care team (GP, Nurse Practitioner, Dietitian, Nurse, Clinical Pharmacist etc).

Referring yourself
If you are wanting to refer yourself please complete the form below or email us at
referral@aotearoadiabetescollective.org.nz We will make contact with your healthcare team on your behalf to request a formal referral.
Referring from primary care
If you are referring from primary care, please refer via Specialist & Referrals from your practice management system or alternatively you can email the referral to us

If you are referring from a specialist service / hospital department please send a referral to us at

Please complete the form below. We don't require all of the information, however the more information you can provide us in the referral, the faster we can process it and organise an appointment.
Your private information
Q. Where does my information go?
A. Referrals are received by Aotearoa Diabetes Collective via email if you complete the online form or via a special referral system called Specialists & Referrals.
Q. How is my information stored/transferred?
A. If you are accepted to our service, then your information is stored in our practice management system (PMS) Elixir that is stored on a secure server. Elixir allows us to send letters to your GP securely via a system called HealthLink.
Q. Who can access my information?
A. The only people who will access your information are people who are directly involved in your care. Administrative staff who schedule appointments do not have access to clinical notes and results.
Q. Do you share my information?
A. We work in partnership with your GP and/or Specialist diabetes service. Like most specialist services, we will send comprehensive letters to your health care team and may request further information from them to help us to provide you with the right care.
You have the right to ask for for a copy of any personal information we hold about you, and to ask for it to be corrected if you think it is wrong. If you’d like to ask for a copy of your information, or to have it corrected, please contact us at admin@aotearoadiabetescollective.org.nz
